Baldur’s Gate 3 has a diverse cast of characters with several classes, races, and backgrounds to choose from, but the Slack-Skinned Head stands out because she’s just a floating severed head. Where is Slack-Skinned Head?

You can find the Slack-Skinned Head inside the Necrotic Laboratory of the Mind Flayer Colony during Act 2. You’ll find her placed on some sort of pedestal next to a contraption called the Mind-Archive Interface.

To speak with the NPC, you’ll need to place either the Willing Mind, True Mind, or Waking Mind in the Mind-Archive Interface. Your exchange with Slack-Skinned Head will depend on which item is in the machine when you interact with her.

Both the Willing Mind and True Mind items can be found laying around the same room as Slack-Skinned Head, but the Waking Mind requires you to solve the brain puzzle in another room of the Mind Flayer Colony.

Who is Slack-Skinned Head?

Slack-Skinned Head was a Zerai warrior who fought against the Illithids, who she refers to as the Ghaik, with psionic abilities. Her people were destroyed by the Illithids and she was kept alive as some sort of trophy.

She will reveal these details to you when you speak to her with the Waking Mind set inside the Mind-Archive Interface. The NPC will then offer to give you her abilities in exchange for purging her mind, essentially killing her.

Accept Slack-Skinned Head’s offer and you will obtain the Githzerai Mind Barrier, Psionic Mastery, Mind Link, and Chaos Influence. Choose to keep her alive, and it may impact later events in the game’s story. We won’t spoil you on what happens, as you might like to see it for yourself.
