So, you're looking at your PS4, knowing full well that it may not make it to the PS5 launch date. The fans sound brutal, things aren't as perky or smooth as they once were.

How to fix a disc read error on PS4
There are a few different fixes for this issue, and that's because there can be a few different causes.
The first port of call should be to clean the disc itself. Use a very soft cloth to do so or you'll risk scratching it and potentially making the issue worse.
If this doesn't work, then you should test out another disc to make sure it's the PS4, and not the disc itself. If that's the case, then you'll have to access safe mode.

To access safe mode on your PS4 you'll need to turn off the console first, then turn it on by holding the power button until you hear a beep after seven seconds of holding it.
The PS4 will now be in safe mode, so you'll have to connect a PS4 controller to it via USB cable. You can then choose option 5 to Rebuild Database. This basically clears some of the clutter from your PS4.
If that doesn't work, you can try Reinstalling System Software, which is also accessed from safe mode and is option 7. This will delete all of the information on the hard drive though, so you'll need to reinstall literally everything including the basic System Software.
If that still doesn't work, then you'll need to send your console in for a service, which you can do by going to the fix and replace page on the PlayStation site.
