Yes, you can have multiple romances in Baldur’s Gate 3. That being said, you won’t be able to just start a relationship with every NPC in the game.

While Halsin and Minthara are pretty much down for whatever, Shadowheart and Astarion may change their minds on open relationships during Act 2. Meanwhile, no one seems to mind if you romance the Guardian.
